Whether you are a patient or provider, our release of information process ensures that you have access to medical records or amend your personal health information in a timely manner all without sacrificing privacy.

To request your medical records, please fill out an authorization form. Once completed, you may return it to our medical records team in person, by email, fax, or mail. If you have specially protected health information (Mental Health, HIV/AIDs, STDs, or Drug/Alcohol) in your chart, please make sure you check off the appropriate area on the form. It may take up to 15 business days for us to process your request for records. You may incur a cost for the copying of records. If you have any further questions, you may call our release of information coordinator at 425-821-2000 x 1569.

For Healthcare Providers:
You may fax your written request to us at 425-458-1708. Please allow up to 15 business days for us to process your request.
